Focused Well-being Services is a support provider for NDIS participants. Our aim is to recruit exceptionally dedicated, knowledgeable, person-centered support workers who is willing to delivers NDIS services to NDIS participants. Our goal is to establish a model of service delivery that focuses on improving the quality of life of our participants.

PURPOSE OF THE POSITION
To join a team that supports individual NDIS participants to achieve an improved quality of life by providing active support and assisting participants to work towards their NDIS Plan goals.
JOB DUTIES
- To encourage participation in community access, social and civic activities.
- To support NDIS participants in their own homes (includes cleaning, cooking, laundry, etc.).
- To keep suitable records so that legislated reporting requirements are met.
- To use your vehicle to drive the participant to appointments and assist with other travel and transport arrangements.
- Be accountable and responsible for decisions and actions.
REQUIRED ABILITIES, KNOWLEDGE, SKILLS, AND PERSONAL QUALITIES
Ability to stay Covid Safe at home and in the community.
Ability to follow policies and procedures established by the provider with attention to detail and due diligence.
Ability to understand the participant’s NDIS Plan goals and record details about progress towards those goals.
Ability to apply good judgement and be alert to potential hazards in the various environments encountered and to respond appropriately in an emergency.
Ability to learn to safely and confidently work alone with the participant and encourage them to try new things.
Ability to learn about the participant and apply that knowledge, including a willingness to facilitate the individual’s choice and control over their own life.
Knowledge, skills and ability to implement the Active Support Model for working with a person with a disability.
Basic knowledge of State and Federal legislation pertaining to Disability, Workplace Health and Safety, Human Rights and the National Disability Insurance Scheme (NDIS) and an understanding of your requirements as a support worker to adhere to workplace policies and procedures established by Focused Well Being Services that reflect the above-mentioned legislative requirements.
SELECTION CRITERIA
MANDATORY
A current unrestricted Australian driver’s license
A current Queensland Blue Card
A current NDIS Worker Screening Check
A current First Aid and CPR certificates
A Certificate 3 in a Disability Support Services qualification or a willingness to complete one.
NDIS module completion certificates for the following modules:
- Worker Orientation Module
- Supporting Safe and Enjoyable Meals Module
- New Worker NDIS Module
- Supporting Effective Communication Module
Demonstrated ability to provide active support to people with disabilities or demonstrated ability in a similar role.
Capacity to rapidly acquire knowledge about the participant and apply that knowledge to provide positive quality of life outcomes.
Knowledge of or the capacity to acquire knowledge of disability sector policies and procedures established by Focused Well Being Services that are governed by relevant State and Federal Legislation
Skill and ability to record accurate data as required by Focused Well Being Services in order to support NDIS planning, implementation and review activities.
Must have a reliable and insured vehicle to use for shifts.
