Join our team as a part-time Support Coordinator and make a real difference! Reporting to the Support Coordination Leader, you'll provide person-centred support to customers living with MS, Motor Neurone Disease, and other neurological conditions. Your role is all about empowering individuals by connecting them to the services and support they need.
This is a permanent part-time opportunity based in Hervey Bay 4 days a week , with the flexibility of working from your home office and attending some community and stakeholder meetings locally. Be part of a team delivering adaptable and accessible services that truly make an impact!

Connect & Support : Assist customers and families by linking them to relevant community services, identifying needs, and advocating for positive outcomes, including crisis support.

Empower & Train : Work collaboratively with customers to build capacity, set goals, and strengthen networks, promoting choice, control, and independence.

Tailored Solutions : Ensure services are person-centered, advocate for funding packages aligned with customer aspirations, and prepare for NDIS planning and reviews.

Collaborate & Build Partnerships : Develop strong relationships with stakeholders, service providers, and community networks to enhance care standards and referral pathways.

Monitor & Improve : Manage documentation and reporting to meet quality standards, track progress, and identify opportunities for improvement and innovation.
Your Winning Qualities:

Qualifications & Experience : Relevant qualifications or equivalent experience in disability or human services, with 3-5 years’ expertise in community-based support services.

Skills & Knowledge : Strong communication, interpersonal, and problem-solving skills, plus knowledge of MS, neurological conditions, NDIS, and the local health sector.

Compliance & Systems : Competence in using Microsoft Office, CRM systems, and adherence to required clearances (e.g., NDIS Worker Screening).

Flexibility & Mobility : Valid driver’s license and ability to travel within Queensland as needed for events, training, and community engagement
