Carrington Care is seeking a passionate and experienced Dementia Care Coordinator to join our team Full Time and lead dementia-specific services across our community. This is a unique opportunity to play a key role in enhancing the lives of residents living with dementia through person-centred, high-quality care.

About the role
This is your opportunity to lead with purpose, shaping dementia care that empowers residents, supports families, and inspires staff. As the Dementia Care Coordinator, you will:
- Lead the delivery of dementia care services across Carrington Care
- Develop and implement individualised, person-centred care plans
- Support and educate staff in dementia care best practices
- Collaborate with multidisciplinary teams and external specialists
- Foster a dementia-friendly environment and culture
- Engage families, provide emotional support, and facilitate support groups
About you – Our Criteria
You’ll bring a blend of clinical expertise, compassionate leadership, and a commitment to delivering person-centred dementia care that supports dignity, independence, and quality of life.
- Registered Nurse qualification
- Minimum 2–3 years’ experience in dementia care (aged care or similar setting)
- Expertise in dementia care principles and behavioural strategies
- Strong communication and leadership skills
- Experience in mentoring and training staff
