The role: As the Clinical Care Consultant at ACH Group’s residential care home Colton Court in McLaren Vale, you will play a pivotal role in coordinating and leading the clinical and customer care services for the residents. You'll be a trusted primary point of contact for residents and their families, ensuring their needs are met with empathy, professionalism, and excellence in this full-time role.
What you’ll do:
- Lead the delivery of high-quality, person-centered care services in line with legislative and professional standards.
- Coordinate individual care plans and clinical services for the residents of Colton Court.
- Respond to feedback and address care needs, in consultation with a multi-disciplinary team.
- Collaborate with the home leadership team to meet compliance standards and support business and service goals.
- Foster strong, respectful relationships with residents, families, and care staff.
- Coach and mentor Nurses, Care Workers, Students and other members of staff
- Actively contribute to a culture of continuous improvement and teamwork.
About you!
- Registered Nurse qualification with current AHPRA registration.
- Significant post-graduate nursing experience.
- Experience working within the aged care environment is advantageous.
- Proven experience clinical leadership role / in the supervision of staff.
- Excellent communication and problem-solving skills.
- Strong understanding of compliance, quality standards, and person-centered care principles.
- Knowledge and understanding of the Aged Care Quality & Safety Standards, legislative and compliance requirements, and AN-ACC funding model.
- High level of clinical care and documentation competence.
- A team player who thrives in a collaborative, dynamic environment.
- A satisfactory Police Check for working in Aged Care less than 12 months at commencement.
