Starting Salary: $76,187.23 + 11.5% Super + Salary Packaging up to $18,550 Tax-Free

About the Role
As a Case Manager, you'll deliver person-centred home care services to clients throughout the Hobart region. Working closely with clients, their families, and healthcare professionals, you'll assess needs, develop personalised care plans, and coordinate high-quality services that align with each client’s goals and preferences.
You’ll empower clients to make informed decisions about their Home Care Packages and ensure their services are tailored to enhance wellbeing and independence.
What You'll Bring
We’re looking for someone who is passionate about health and wellbeing, excels at building relationships, and thrives both independently and as part of a team.
To be successful, you’ll have:
- Certificate III or higher in Case Management, Aged Care, Disability, Community Services, or a related field
- Previous experience in case management within aged care, disability, or youth services
- Excellent communication and interpersonal skills
- A valid driver’s licence and access to a reliable vehicle
- Confidence to work autonomously while collaborating with internal and external stakeholders
