About the Role:
Fix-Term, Full-Time role providing comprehensive social work ser to patients, families and carers in our rehabilitation team.
Conduct psychosocial assessments for patients and assess issues of risk
Drive patient care and safe discharge planning including referral to community services, short and long-term accommodation/ placement for patients.
Provision of counselling, individual and family, to facilitate adaptation to medical conditions, lifestyle implications and issues of anticipatory grief and loss.
Advocate for patients, families and carers as appropriate including where required, applications to NCAT: Guardianship Division and the National Disability Insurance Scheme (NDIS), Housing NSW, Department of Home Affairs, My Aged Care (MAC), LTCS, facilitate CTP claims and other Government and NGO entities as required.
Selection Criteria:
Degree in Social Work and eligible for membership of the Australian Association of Social Workers (AASW) with evidence of clinical experience in a hospital or health related field as a practising Social Worker.
Demonstrated clinical skills and knowledge in psychosocial assessment, counselling, therapy interventions, case management, advocacy and discharge planning.
Knowledge in one and/or clinical experience in rehabilitation across both within an inpatients and outpatients setting.
Demonstrated experience and ability to work as a member of multidisciplinary teams in a high-pressure environment with competing demands with sound negotiation and liaison skills.
Well-developed time management, prioritisation, computer, written and verbal communication skills.
Ability to participate in an extended hours on call service.
