Optimal Ageing Service Coordinator – Empower Independence and Wellbeing for Older Adults
Are you passionate about supporting frail, aged individuals to live independently and improve their quality of life? We are seeking a dedicated and compassionate Optimal Ageing Service Coordinator to join our team and ensure the delivery of client-directed care to promote the health and wellbeing of our clients.
As the Optimal Ageing Service Coordinator, you will play a key role in coordinating services for frail, aged individuals under a client-directed care model. Your responsibilities will include prioritising referrals, conducting assessments and reviews within the client’s home, and developing personalized care plans to support their independence. You will also be responsible for rostering, supervising community support workers, and maintaining a waiting list to ensure services are provided in a timely and efficient manner.
In this role, you will work closely with clients, their families, professional staff, and other home support agencies within the region to ensure seamless care coordination. Your efforts will directly contribute to helping clients remain in their own homes, living as independently and comfortably as possible.
This position requires excellent organisational and communication skills, as well as a strong commitment to providing high-quality, person-centred care. Applicant Information
This is a part time vacancy, working 45 hours per fortnight and salary will be adjusted according to actual hours worked.
Appointment will be subject to a satisfactory Criminal History Check and appropriate immunisation requirements.
