Job position: Disability Support Worker

Indicative Salary/wages: AUD 69,000 - $80,000 / year + superannuation, pending agreed pattern of work hours across 7 days per week, in accordance with terms and conditions of HomePlace’s Enterprise Agreement

Location: 19-21 Belmore Terrace, Woodville, South Australia, SA 5011

Contract type: Full-time based

Hours: 38 hours a week for at least 4-year term

Reporting to: Manager, Participant Services and Quality

HomePlace is a disability organisation. We are a registered provider with the NDIS. We started in 1989 to help people living with an intellectual disability to live as safely and independently as they can in places they are proud to call their home. We are a small organisation. The ‘HomePlace Way’ is based on our team getting to know each person for the unique individual they are. Each participant’s goals, interests, work, community activities, support needs, form a unique picture of how each participant wants to live their life on their terms.

We have a lot of experience in supporting people living with disability and a range of challenges that impact their life and independence. This includes people living with intellectual disability/cognitive issues, physical disabilities, mental and other health challenges.

The Role Job Responsibilities:

1. In line with HomePlace values and principles, this position focusses on:

• Assisting participants with identifying and achieving their personal goals.

• Empowering participants through person-centred active support to live life in the way that has meaning for them, and if possible and wanted by the participant, to actively belong in their community

• Supporting, encouraging and assisting participants in developing their home to reflect their individuality.

• Personal care service involving teaching, monitoring, assisting with or performing some or all self-care activities including:

• Toileting, showering and/or bathing

• Dressing, grooming and other personal hygiene tasks

2. Health Care

• Accurately document and report all health care information and issues.

• Support Individuals to attend Health care appointments.

• Follow health care procedures as directed by a Health Care Practitioner.

• Ensure safe administration of medication consistent with HomePlace procedures and protocols.

3. Teaching and modelling appropriate social skills.

4. Housekeeping :

• Teaching, monitoring, physically assisting or performing any task that would normally be undertaken in any household. This includes the full range of tasks needed for:

• Laundry, cleaning, cooking, shopping, sewing

• Gardening, household maintenance and upgrade

• Shopping

5. Recreation and Leisure

• Research suitable and age appropriate recreation and leisure pursuits in line with HomePlace objectives for the inclusion of participants into ordinary community life.

• Develop a range of options that are likely to be enjoyable and fun for the individual concerned.

• Ensure that a range of recreational activities are suitably planned, organised and attended.

6. Financial Management

• In keeping with HomePlace’s Management of personal finances’ policy, maintain accurate personal and household financial records, where appropriate.

• Ensure that the individual’s personal spending is consistent with their financial goals and choices and arrangements negotiated with their guardian/financial administrator.

7. Personal Relationships and Social Networks

• In line with HomePlace policies, vision and core support themes, ensure that all possible effort is made to expand and nurture a range of “non-paid” relationships in the lives of participants.

• Ensure that accurate and thorough records are maintained that document family details, close personal relationships and social networks in an individual’s life.

• Ensure that participants are assisted to be introduced and supported to participate in their neighbourhood and community.

• Assist participants to develop and improve their communication and relationship skills.

8. Behaviour Support

• Support Workers are expected to be familiar with the HomePlace policy on Behaviour Support and to follow the methods of intervention preferred by HomePlace as contained in this policy and any behaviour support plans.

• Accurately document and report as required.

• Assist participants to develop the skills needed to assist them to manage their own behaviour in more adaptive ways.

9. Day Services and Employment Options

• Ensure effective information exchange between HomePlace and other services providing support to participants.

10. Family Liaison

• Assist with building close relationships between HomePlace and families.

• Assist family members to maintain ongoing contact with the individual concerned.

• Ensure that enquiries and concerns are followed up promptly.

11. Administration

• Effectively record organisational, individual and household information including:

• Entering in with each participant’s input, notes of each session in the participant’s “GOMs” section of their MYP file

• File and support notes, journals and diaries, charts and checklists

• Financial records

• Medication records

• Household, maintenance, and tenancy records

• Orientation information

• Neighbourhood and community resources

• Assist with organisational administrative duties as required, ensuring compliance with HomePlace’s policies and procedures.

• Support HomePlace’s or relevant teams with projects as needed, contributing to efficient service delivery and continuous improvement initiatives.

12. Team Membership:

• Develop and encourage positive team relationships and open communication in the support of each other and of the people HomePlace supports.

• Attend scheduled team meetings, support meetings, review and training sessions as required.

• Actively promote a positive team environment.

• Assist with effective teamwork in the implementation of service plans for participants.

• Provide information, assistance and guidance to other team members and assist with new team member orientation.

13. Communication:

• Actively participate in communication and information sharing with colleagues and management to facilitate effective HomePlace operations.

• Consult collaboratively and respectfully with participants and/or their families or representatives at all times.

14. Health, Safety and Welfare

• Comply with and adhere to all HomePlace Safety and Injury Management policies and procedures and take all reasonable care that your actions do not impact on the health, safety or wellbeing of others at the workplace.

Skilled and experienced requirements:

To be successful in this role you will need to demonstrate your ability to meet the selection criteria outlined below:

• Achieve the purpose of the position through providing support to people who are assisted by HomePlace, ensuring our participants are assisted to create opportunities for improvements in their quality of life as well as to be safe and well cared for.

• Manage your time between the support needs of participants, including their social and lifestyle needs, household organisation and housekeeping tasks, and communication responsibilities to the participant, their family and other members of the team.

• Have experience working with people with intellectual disability/cognitive impairment and their support networks.

• Having the Certificate III in Disability, Individual Support, Aged care or similar

• Have demonstrated experience in providing person-centred active support

• Have a sound understanding of the National Disability Insurance Scheme.

• Have high level of both verbal and written communication, interpersonal, and negotiation skills.

• Demonstrate a high level of computer literacy and skills.

• Have strong writing and problem solving skills

• Have proven experience in record keeping, including for multiple participants with complex needs, including an ability to maintain confidential information.

• Have knowledge and commitment to NDIS Code of Conduct, Practice Standards and Quality Indicators

• Have demonstrated ability to be able to work independently and as part of a collaborative team.

• Have a current unrestricted South Australia driver’s license.

• Have a aurrent Emergency First Aid Certificate (Basic Emergency Life Support/senior first aid certificate)

• Have a current NDIS screening clearance.

• Have an up to date COVID vaccination status
