Exciting Career Opportunity: Community Support Worker at Eyre and Far North Local Health Network

The Eyre and Far North Local Health Network (LHN) is seeking a dedicated Community Support Worker to join our team and play a vital role in supporting consumers to maintain their independence in a safe, person-centred environment.

In this role, you will be an integral part of the Consumer Directed Care model, which empowers consumers to have greater control over their lives. Consumers are encouraged to participate in identifying and setting goals, deciding on the services they wish to receive, and choosing their preferred providers.

As a Community Support Worker, you will provide essential services such as personal care, cleaning, meal preparation, transport, and social support. You will work closely with consumers who are aged, frail, or disabled, helping them remain in their own homes or supporting them in Residential Aged Care facilities. Your role will directly contribute to their well-being, promoting independence and enhancing their quality of life.

This position requires flexibility, empathy, and the ability to work autonomously in the community. You will collaborate closely with consumers, their families, and other healthcare professionals to ensure high-quality care and outcomes.

What We're Looking For:

  • Certificate 3 in Individual Support (Ageing, Disability or Home and Community).
  • Current drivers licence and willingness to drive.
  • Current police clearances (refer police check clause).
  • Contactable by telephone.
  • Access to a reliable, registered and insured (minimum of third-party insurance) motor vehicle.

Applicant Information

This is a part time vacancy, working 37.5 hours per fortnight and salary will be adjusted according to actual hours worked.
