Northern Health are seeking applications for a motivated and suitably experienced Diabetes Nurse Educators to join the Hospital in the Home team – Hospital in the Home Program.
About Northern Health:
Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.
Services are provided through our five main campuses:
Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn Centre, and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.
About the Role:
The Diabetes Nurse Educator provides comprehensive, patient-centred care to individuals with diabetes and their carers, and education of team members about diabetes management within the Hospital in the Home program (HITH).
This role requires the individuals to be highly motivated, organised and work well within a team.
This is an Ongoing Full-time position, Monday - Friday 0830 – 1700.
To be successful in this role you will have:
- Experience and good working knowledge of Type 1, Type 2 and Type 3c diabetes management.
- Assess, plan and implement a suitable education program, individualised to suit the client with diabetes, including the family and significant others, where possible.
- Familiar with the use and application of diabetes technology such as Continuous Glucose Monitor as well as insulin pumps.
- Proven ability to work autonomously and in a team-based environment, with a high level of problem solving and planning ability.
- Experience working with clients from culturally and linguistically diverse backgrounds.
- Current Australian Diabetes Educators Association credentialing status or working towards this.
