South West Healthcare (SWH) is seeking a passionate and dedicated Aboriginal Liaison Officer - Chronic Health to join our team and help improve the health outcomes of Aboriginal and Torres Strait Islander people living with chronic and complex conditions.
About the role
This important position supports clients by promoting early intervention, self-management, and culturally safe care pathways for those living with chronic illness. Working collaboratively with hospitals, community health, aged care, and Aboriginal Community Controlled Health Organisations (ACCHOs), you will play a vital role in improving care coordination and reducing unnecessary hospital visits.
As the Aboriginal Liaison Officer - Chronic Health, you will:
- Advocate for Aboriginal patients across acute and emergency settings.
- Collaborate with chronic disease clinicians to promote culturally appropriate care and education.
- Support clients and families in developing and engaging with goal-oriented, person-centred care plans.
- Foster partnerships between SWH, ACCHOs, and other service providers.
- Provide tailored health education and encourage active participation in self-management programs.
- Assist with capacity building across SWH by participating in education, training, and quality improvement initiatives.
Position overview
- Full time - On-going position (80 hours per fortnight) with a monthly Accrued Day Off
- Warrnambool Campus
- Community Development Worker Class 2
- Salary Range: $67,538 - $92,227, per annum, plus superannuation
What you bring
- Relevant qualifications in Nursing, Allied Health, or Aboriginal Health, with current AHPRA registration (as applicable).
- Strong skills in assessment, care planning, and service delivery.
- Proven ability to work independently and within a collaborative team environment.
- Excellent communication skills in high-pressure, multidisciplinary settings.
- In-depth understanding of community-based supports and networks.
- A minimum of 5 years' experience in an acute or community health setting.
- Advanced skills in time management, planning, and prioritising tasks.
