The Location
Located in Brisbane’s Bayside area of Thornlands, Bolton Clarke’s Moreton Shores is a 120 bed Residential aged care home providing s high-quality customer service and care to our customers.
The Role
As a Clinical Coordinator, you will ensure our aged care clients and clinical employees are supported. Combining case management, clinical governance, ACFI, rostering, and staff management, the role of Clinical Coordinator is essential to ensure the leadership and coordination of all aspects of clinical care.
Key Accountability
- Provide clinical leadership and supervision of care teams including Nurses, Personal Carers & Allied Health staff
- Develop care plans in consultation with the resident, family/carer and other members of the care team
- Promote a culture of service delivery that aims to improve the lifestyle experiences of residents
- Maintain an information system that supports and enhances the provision of accurate documentation relating to the resident
To be successful, you will have:
- Current Division 1 Registered Nurse registration with AHPRA
- Minimum 2+ years’ experience as a Registered Nurse
- Demonstrated experience in clinical governance, accreditation and ACFI
- Valid working rights in Australia (Passport, Birth Certificate etc)
- Background checks and references
- Current flu season vaccination (or willing to obtain)
- Evidence of Covid-19 vaccination
- NDIS Clearance (or willingness to obtain prior to commencement)
- Qualifications / AHPRA
