We are a not-for-profit National Disability Insurance Scheme (NDIS) provider. Our Head Office is based in Wagga Wagga but we are located throughout the Riverina Murray region.
About The Opportunity
We’re looking for a passionate and experienced individual to join our Lifestyle Choices team in Wagga Wagga. This is a full-time role, operating within business hours between Monday and Friday each week and is located at the picturesque Hildasid Farm.
Responsibilities
As a Program Coordinator, you’ll be responsible for the lifecycle of client’s NDIS plans at Lifestyle Choices, including implementation and ongoing coordination. You’ll work closely with staff, clients, families, and other key stakeholders to achieve positive outcomes and meaningful experiences for those we support. Your responsibilities will include:
Develop personalised programs designed to support clients to achieve their goalsMaintain client records and track progress towards goals by performing scheduled reviews, regular check ins and compiling core reportsProvide guidance and mentorship to a team of Community Support Workers, developing their skills and increasing our capability to provide supportsPromote and provide hands on person centred support to clients as required

This position is remunerated at $83,486 annually plus super, annual leave loading & NFP salary packaging.
