Job description:About the Role Closing Date: 20/05/2025 Join the largest regional health service between Sydney and Melbourne Dynamic work environment and collaborative team culture Situated at the foot of the Aussie Alps and on the banks of the mighty Murray River The Regional Palliative Care Consultancy Nurse Practitioner will be a valued member of the East HUME Regional Palliative Care Consultancy service. The Nurse Practitioner will join the team of Palliative Care Physician & Registrar, Nurse Practitioner Candidate in Palliative Care & Grief & Bereavement Worker. The team travel across the east Hume region to provide primary health care providers in community & hospital settings, & their patients, with specialist palliative care clinical consultation to enhance the quality of life, alleviate suffering & support the holistic needs of patients & their families. The team provide strong clinical leadership for palliative care across the region & through delivery of education & training, clinical mentoring & driving improvement in clinical safety & quality. We are actively seeking candidates who meet the following requirements to apply for this exceptional opportunity. Current registration & endorsement with AHPRA as a Nurse Practitioner of Palliative Care Demonstrated ability to work autonomously & collaboratively in an advanced & extended clinical role. Demonstrated capability to communicate effectively with a range of divergent health care professionals Demonstrated ability to engage with & promote reflective practiced across the nursing workforce Demonstrated ability to apply & promote evidence informed practice at point of care
