As an Outreach Case Manager in our Hume Program , you’ll work alongside individuals and families who are homeless or at risk, supporting them to navigate housing pathways, access vital services, and build stability for the future.
Using a team-based case management approach, you’ll be part of a collaborative, values-driven team where your insights and contributions are genuinely valued. Much of your work will take place in the community, meeting people where they are—whether in temporary accommodation, their homes, or other safe spaces. You’ll play a key role in empowering them to achieve their goals, increase self-reliance, and find lasting stability.
ABOUT YOU
You’re passionate about social justice and committed to making a difference. You bring experience in case management or a related support role and understand the impact homelessness has on individuals and families.
You’ll ideally have:

A relevant tertiary qualification (e.g. social work, community services, youth work) or extensive experience in a similar role

Strong interpersonal skills and the ability to engage effectively with people from diverse backgrounds

Confidence working independently and as part of a collaborative team

Excellent organisational skills with the ability to manage competing priorities and maintain accurate case notes

Familiarity with the homelessness sector and frameworks such as the Opening Doors model (preferred but not essential)
