About the roleWe are seeking a highly qualified and experienced Registered Nurse to join our team in the pivotal role of Care Coordinator. The successful candidate will have a minimum of five years' experience in acute and/or community healthcare settings and have a strong commitment to patient-centred care.
As a Care Coordinator, you will play a key role in enhancing the quality of care for individuals living in the community with chronic and complex health conditions. Your primary responsibility will be to prevent avoidable hospital admissions and reduce both the frequency and duration of hospital stays.
You will collaborate closely with a wide range of healthcare providers—including general practitioners, specialists, community health, aged care, and acute services—to ensure continuity of care and to streamline health pathways for clients.
Position overview
* Employment Type: Part-time (64 hours per week), On-going position
* Location: Camperdown Campus
* Classification: Community Health Nurse
* Salary Range: $107,390.40 pro rata, plus superannuation
What you bring
* Current registration with AHPRA as a Registered Nurse.
* Minimum of five years' clinical experience in acute and/or community settings.
* Demonstrated experience in care coordination, chronic disease management, or case management.
* Strong understanding of healthcare systems, referral pathways, and multidisciplinary collaboration.
* Excellent communication, problem-solving, and organisational skills.
* Ability to work independently and as part of a dynamic team.
