About Strive
Founded in 1996 as Landsdale Family Support Association Inc., our original purpose was to provide respite camps for children with disabilities at Landsdale Farm School. In 2009 we changed our name to Family Support WA Inc, as we expanded our services to include young adults. Over the last 25 years we have grown to provide several services to people of all ages with disabilities, as well as now operating the Landsdale Farm located in the northern metropolitan suburb of Darch. We became Strive Community Services Inc., in 2021 to better reflect the wide range of services we provide in the community.
About the Opportunity
We are seeking a motivated Team Leader to oversee the delivery of high-quality support services to clients based at our Supported Independent Living (SIL) house in Baldivis. This role focuses on managing day-to-day operations, leading a team of support workers, and ensuring compliance with regulatory requirements and the organisation’s policies and procedures.
Key Responsibilities:
- Coordinate client medical appointments, prescriptions, and medication administration.
- Conduct house maintenance audits and ensure compliance with safety standards.
- Review client case notes and manage shift operations.
- Address and resolve service delivery issues promptly.
- Oversee client services and goal progress, providing guidance to support staff.
- Conduct one-on-one meetings to review staff training needs.
- Promote a positive team environment and ensure effective communication.
- Manage client activity planners and staff rosters.
- Ensure accurate client files and maintain open communication with stakeholders.
- Support Senior Team Leaders with operational needs.
- Conduct fire drills and house inspections, ensuring a safe environment.
- Build strong relationships with clients and families, providing necessary support and seeking feedback.
Selection Criteria:
- Certificate III in Individual Support, preferably a Certificate IV in Disability.
- 2-3 years' experience in disability services, preferably in a leadership role.
- Strong working knowledge of NDIS operations and person-centred planning.
- Excellent communication, organisational, and team management skills.
- WA driver’s license and willingness to travel.
