Are you passionate about supporting individuals with disabilities and enabling them to live independently with dignity? We are seeking a dedicated Community Support Worker – Disability to join our team. This is a rewarding opportunity to make a positive impact on the lives of consumers by providing personalized care and support in line with the Consumer Directed Care model.
About the Role:
As a Community Support Worker – Disability , you will play a key role in delivering personalized services, such as personal care, cleaning, meal preparation, transport, and social support, to consumers who are aged, frail, or disabled. You will work directly with consumers, their families, and other health professionals to develop and implement care plans that promote independence and well-being. You’ll work autonomously in the community, ensuring that each consumer receives the services they need to stay in their own home or a residential aged care facility, as they actively participate in managing their care.
In this role, you’ll be integral to the Consumer Directed Care model, where consumers control their care funding and have the freedom to choose their services and providers. You’ll need to be flexible, empathetic, and conscientious as you collaborate with consumers to deliver responsive care, helping them achieve their goals and maintain independence.

What We’re Looking For:
- Certificate 3 in Individual Support (Ageing, Disability or Home and Community).
- Current drivers’ licence and willingness to drive.
- Current police clearances (refer police check clause).
- Contactable by telephone.
- Access to a reliable, registered and insured (minimum of third-party insurance) motor vehicle.
