Our client, a reputable aged care provider, is seeking an experienced Aged Care and Home Care Coordinator to join their Blackburn office on a full-time, two-month contract, with the potential for extension. This office-based role offers a fantastic opportunity for a dedicated professional to support clients in maintaining their independence at home.
About the Role
The successful candidate will play a key role in coordinating and supporting clients receiving Home Care Packages. This position involves client intake, assessments, ongoing reviews, and ensuring that clients receive appropriate care and services.
Key Responsibilities
  • Provide Home Care Package clients with relevant information and service options.
  • Conduct client intake, assessments, planning, and regular reviews.
  • Support clients in understanding their package budgets and available choices.
  • Maintain accurate case notes and communicate effectively with internal and external stakeholders.
  • Work within legislative and compliance requirements while respecting client rights.
  • Promote and refer clients to additional services where appropriate.
  • Implement best-practice models in care assessment, planning, and evaluation.
Key Requirements
  • Must have experience in aged care and Home Care Packages.
  • Strong case management and care coordination experience.
  • Excellent communication and stakeholder engagement skills.
  • Ability to work within compliance and regulatory frameworks.
  • A commitment to person-centered care.
