Our client, a reputable aged care provider, is seeking an experienced Aged Care and Home Care Coordinator to join their Blackburn office on a full-time, two-month contract, with the potential for extension. This office-based role offers a fantastic opportunity for a dedicated professional to support clients in maintaining their independence at home.About the Role
The successful candidate will play a key role in coordinating and supporting clients receiving Home Care Packages. This position involves client intake, assessments, ongoing reviews, and ensuring that clients receive appropriate care and services.
Key Responsibilities
- Provide Home Care Package clients with relevant information and service options.
- Conduct client intake, assessments, planning, and regular reviews.
- Support clients in understanding their package budgets and available choices.
- Maintain accurate case notes and communicate effectively with internal and external stakeholders.
- Work within legislative and compliance requirements while respecting client rights.
- Promote and refer clients to additional services where appropriate.
- Implement best-practice models in care assessment, planning, and evaluation.
- Must have experience in aged care and Home Care Packages.
- Strong case management and care coordination experience.
- Excellent communication and stakeholder engagement skills.
- Ability to work within compliance and regulatory frameworks.
- A commitment to person-centered care.
- Full-time, office-based role (Monday to Friday, 9:00 AM – 5:00 PM).
