The Role
As the Clinical Care Coordinator,
• You will be responsible for the education, support, and clinical practice of all RNs.
• You will be in charge when the Care Director and Home Manager are away.
• provide and coordinate resident care, and to coach and mentor staff to ensure an effective, efficient functioning team.
• You will perform in accordance with legislation and practice within a professional and ethical framework.
• You will be responsible for assessing and evaluating individual resident care at a systems level to ensure service delivery reflects evidence based best Practice.
• You will work cohesively with the Residential Aged Care Manager and Care Director on clinical service delivery, compliance and audit process, and to act as a clinical resource for staff, where effective leadership and clinical education will feature as a major component of the role
Qualification and Skills
• Current Registration with AHPRA, with no conditions
• Strong clinical background
• Experience working within an aged care or similar environment would be highly regarded.
• Prior ACFI experience
• Previous experience and ability to lead and manage staff in a clinical care setting
• Excellent clinical documentation skills
