The Clinical Intake Manager supports the growth and efficiency of the organisation’s Supported Independent Living (SIL) and accommodation services. The role assists clients with complex and high-intensity support needs to transition smoothly into appropriate SIL care. This includes people with challenging behaviours, complex disabilities, significant health conditions, hospital and nursing home long-stay patients, or those who have faced multiple breakdowns in living arrangements.
This role ensures that client intake is safe, timely, person-centred, and clinically informed. The Clinical Intake Manager works with Support Coordinators, hospital and health services, aged care providers, and other organisations. The role also involves assessing the client’s clinical needs, coordinating care with multiple stakeholders, and ensuring all transitions follow NDIS guidelines and best practices.
Beyond case coordination, the Clinical Intake Manager contributes to strategic planning. They analyse service demand, identify gaps in the intake process, and develop new solutions to improve safety and efficiency. This role requires clinical expertise, safety and quality practices, leadership, and strong stakeholder engagement. The goal is to support organisational objectives and achieve better client outcomes.
The is a dynamic role involving:
- Collaboration through liaising with hospitals, aged care providers, NDIS, and disability services and lead discharge planning.
- Clinical Decision-Making by conducting assessments, develop transition plans, support complex care coordination, and train support teams.
- Compliance and ensure adherence to NDIS standards, oversee compliance, and maintain documentation.
- Stakeholder Engagement by building relationships, conduct meetings, advocate for clients, and collaborate with teams.
- Strategic Thinking to analyse trends, improve intake efficiency, and align initiatives with strategic goals.
- Problem-solving by addressing barriers to transitions, adapt to changes, and apply innovative care strategies.
- Preparing reports, participate in audits, and identify staff training opportunities.
- Nursing Qualifications: Tertiary qualification with current AHPRA membership.
- Postgraduate Qualifications: Case management, disability care, or community health (desirable).
- Experience: Extensive in assessing and coordinating complex client transitions into SIL settings.
- Referral Pathways: Ability to establish and maintain strong connections with key stakeholders.
- Care Plans: Expertise in coordinating individualised plans aligned with NDIS support categories and client needs.
- Clinical Assessments: Demonstrated ability to conduct comprehensive assessments and develop tailored transition plans.
- Support Needs: Strong understanding of high-intensity support, behaviours of concern, and chronic health management.
- Client Outcomes: Proven experience in balancing clinical considerations with lifestyle goals in community settings.
- NDIS Knowledge: In-depth understanding of NDIS legislation, quality standards, and hospital discharge protocols.
- Compliance: Proven ability to develop and oversee compliance and quality assurance measures.
- Risk Management: Experience in risk management, documentation accuracy, and regulatory adherence.
- Communication Skills: Strong skills to engage with Support Coordinators, hospital teams, aged care providers, clients, families, and allied health professionals.
- Case Conferences: Ability to conduct assessments and care planning discussions with multiple stakeholders.
