Position Title: Diabetes Educator
Fixed-term part-time until 30.06.2025 (7 hours per fortnight)
Location : South West Region based in Bridgetown

Position Overview
We are currently seeking a dedicated and passionate Diabetes Educator to join our team on a fixed-term part-time basis (7 hours per fortnight). In this role, you will contribute to our mission by delivering face to face diabetes education to reginal clients in the Warren Blackwood region based in Bridgetown.
Key Components of the Role
• Provide Diabetes Education services tailored to meet the needs of the in-dividual
• Facilitate the development of diabetes self-management skills to improve patient health outcomes
• Develop care plans in conjunction with the patient and their primary health care provider
• Monitor and review patient progress and health status
What you bring
• Tertiary qualification or recognise Certificate in Diabetes Education
• Current registration to professional body
Essential Requirements: To be considered for this role, you must meet the following criteria:
• The right to work in Australia
• A valid WA driver’s license
• A valid and clear police check
• A Working with Children Check (WWC)
