The Opportunity:
We are seeking an experienced Clinical Care Coordinator to join our dedicated team and contribute to the delivery of high-quality clinical care within our facility. You’ll have the chance to guide a dedicated clinical team, shape the delivery of high-quality, person-centered care, and contribute to the overall well-being of our residents. In this role, you’ll lead by example, ensuring that care plans are tailored to meet individual needs, while promoting a culture of excellence and continuous improvement.
Key Responsibilities:
• Lead and manage the clinical care team, ensuring the delivery of high-quality, person-centered care to our residents.
• Develop, implement, and review individual care plans for residents.
• Oversee the day-to-day clinical operations, ensuring that staff are supported and clinical practices are consistent with best practices and industry standards.
• Provide leadership, supervision, and mentorship to nursing staff and other clinical team members.
• Monitor the health and wellbeing of residents, identify changes in condition, and ensure timely and appropriate interventions are taken.
• Collaborate with other departments to ensure holistic care that meets the emotional, social, and physical needs of residents.
• Maintain accurate clinical documentation and ensure compliance with aged care standards, policies, and procedures.
• Contribute to quality improvement initiatives and lead audits and assessments to enhance care delivery.
• Liaise with families and medical professionals to ensure residents' clinical needs are fully understood and managed effectively.
• Participate in training, and ongoing development of our clinical staff.
What we’re looking for:
• 4+ years Registered Nurse (RN) with current AHPRA registration.
• Proven experience as a Clinical Care Coordinator or in a similar senior clinical role in aged care.
• Strong knowledge of the Aged Care Quality Standards and relevant regulations.
• Excellent clinical assessment, problem-solving, and decision-making skills.
• Exceptional leadership abilities with experience managing and mentoring a clinical team.
• Outstanding communication and interpersonal skills, with the ability to build rapport with residents, families, and staff.
• Strong organisational skills and attention to detail.
