Where you'll be working

The position of Complex Care Coordinator sits within Northern Sydney Home Nursing Service (NSHNS) in the Division of Primary and Community Care, Northern Sydney Local Health Service. The care is delivered in the Ryde area. The position is based at Macquarie Hospital, North Ryde.

What you'll be doing

The purpose of the Clinical Nurse Specialist (CNS) position is to provide senior nursing clinical leadership in the development of specialist clinical practice and service delivery within a nursing unit, team or service. The CNS employs advanced clinical skills and exercises extended autonomy of decision making working closely with managers and other senior clinicians.

This position works closely with patients/clients and carers and their General Practitioners (GP) to identify clinical and non-clinical strategies, self-management support, referrals to specialist services or community agencies, and goals to improve wellbeing.

It is the responsibility of this position to facilitate the implementation and evaluation of this shared care plan with the patients/clients and carers and their GPs.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:
  • Current registration as a Registered Nurse with the Australian Nurse Health Practitioner Regulation Authority (AHPRA) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification or such other qualifications or relevant equivalent experience.
  • Experience in practice that promotes self-management support, behaviour change and psychosocial support and a strong understanding of the health and psychosocial issues for people living with chronic conditions and their carers.
  • Demonstrated ability to communicate effectively with patients, carers, and a diverse range of health care providers including those in government, non-government, and community organisations.
  • Substantial and current experience in Complex Care Coordination, using advanced clinical skills in patient assessment and care planning.
