The CCLHD – Clinical Change Project Manager – Community, Chronic and Complex Care, is responsible to the Director Community, Chronic and Complex Care and will work in collaboration with staff and managers to lead the implementation and delivery of key community health care changes or initiatives in alignment with the district’s annual priorities and broader NSW Health strategies.

This position will undertake the full role of clinical change management activities including thorough consultation, project planning, stakeholder engagement, co- design, problem solving, implementation, milestone reporting and evaluation.

The priority areas for Community, Chronic and Complex Care include, but are not limited to, delivering more care in the community, expanding and optimising out of hospital of care, developing alternate care pathways to reduce unnecessary admission to Emergency Department, better integrating care between hospital and community and supporting early discharge strategies.
