- Conduct assessments to determine client needs and develop tailored support plans

- Collaborate with community organizations and service providers to facilitate client access to services
- Document client interactions and maintain accurate records according to confidentiality standards
- Participate in training and professional development opportunities to enhance service delivery
- Qualification in social work, psychology, welfare, or a related field
- Experience working in welfare support or social services is highly desirable
- Strong interpersonal and communication skills
- Ability to work with diverse populations and understand their unique challenges
- Excellent organizational skills and attention to detail
- Compassionate and empathetic approach to client interactions
- Knowledge of local resources and community services is a plus
