More Than Support is a NDIS registered service provider operating in Victoria and Queensland. Our company provides a variety of services including personal care, mobility assistance, nursing care, cooking and meal plan services, travel and transportation, household tasks, community participation, therapeutic support, specialised disability accommodation, supported independence living, support coordinator (level 2 and level 3), counselling and psychology.
About the role
We are looking for a passionate and caring NDIS Support Coordinator to join our dedicated team in Melbourne and make a difference in our clients' lives. As a Support Coordinator, you will help our clients achieve their personal goals and fully participate in the community, while creating connections between clients and service providers. This position operates in a dynamic environment using a multidisciplinary approach to provide an integrated and flexible model of care.
To be successful for this position you will;
- Work closely with clients to identify their needs, personalise support, and ensure they are receiving appropriate services.
- Meet with participants to formalize a support plan and help them engage with different NDIS providers and health professionals.
- Promote and foster independent decision-making, including choice and control, to ensure the participants achieve their goals.
- Coordinate and liaise with service providers, review goals, and work with participants to determine new ones.
- Maintain positive working relationships with clinical and other allied health services, as well as non-government bodies and disability associations/groups.
- Review NDIS plans and assist in forecasting, ensuring that spending remains within budget.
Skills & Experience
- Minimum Certificate IV in Disability (CHC43115) or Diploma of Community Services (CHC52015).
- Strong understanding and experience with the NDIS and the disability sector.
- At least 1 year of experience working with and implementing individual NDIS plans.
- Prior experience in Support Coordination or demonstrable experience in Community Sector Case Management will be highly advantageous.
- A self-starter, self-motivated individual who can work independently and takes pride in delivering high-quality services.
- Demonstrated experience working with people with disabilities.
- Excellent verbal communication skills and the ability to engage with participants and their families.
- Strong written and computer skills for recording notes, gathering evidence, writing reports, correspondence, and other documents.
