About the role

An exciting opportunity to join a thriving community healthcare organization with a supportive, values-driven culture and an engaged workforce. We provide primary health, care management, and community support services tailored to individual needs across Melbourne’s Eastern suburbs. We are seeking a dedicated professional to join our team in a flexible, full-time Max term until June 2026 to deliver specialist care finder services to assist older adults with engagement barriers in understanding and accessing aged care and community support. Reporting to the Manager Community Access and Outreach, you will provide intensive support to seniors who struggle to navigate the system independently and help prevent vulnerable individuals from falling through the cracks by connecting them with essential services. What you will be doing

  • Assertive Outreach
  • Engagement and rapport building with potential clients and local intermediaries
  • Support people to interact with My Aged Care so they can be screened for eligibility
  • Support to explain and guide people through the assessment process
  • Support to help people to find the aged care supports and services they need and connect with other relevant supports in the community
  • High level check-in with clients on a periodic basis and follow up support following service commencement

What you will bring Key Selection Criteria

  • Degree in social work or other relevant qualification
  • Experience in assertive outreach and engaging with people who are often reluctant to access support services
  • Experience in supporting clients to understand and navigate service systems
  • Demonstrated knowledge of trauma informed practice and cultural safety and evidence-based approaches to improving wellbeing
  • Experience with building and maintaining effective working relationships internally and with external organisations, networks and community groups
  • Experience in working with clients to identify and support referrals to relevant services

Attributes

  • Strong communication and interpersonal skills
  • Proficiency in spoken languages other than English especially Arabic, Farsi, Cantonese, Mandarin and Vietnamese (desirable)
  • Demonstrated behaviours consistent with Access Health and Community values
