Position Responsibilities and Tasks:
- Conduct interviews with clients to assess the scope and nature of challenges and develop strategies for assistance.
- Evaluate client needs, create personalised plans, and implement educational, training, and support programs to enhance clients’ well-being.
- Track client progress, document improvements, and ensure accurate and timely reporting of outcomes.
- Refer clients to relevant support agencies or services to ensure access to appropriate resources, such as daily living assistance or accommodation support.
- Collaborate with community groups, welfare agencies, government bodies, and private businesses to raise awareness of community resources, services, and support available for individuals with disabilities.
- Assist families and provide educational guidance and care for children, as well as individuals with disabilities, including those in accommodation, group housing, or respite services.
- Support clients with Acquired Brain Injury (ABI) or Spinal Cord Injury (SCI) to achieve independence and improve life skills through tailored programs.
- Providing guidance and strategies to help young people overcome obstacles, such as emotional, social, and financial challenges.
- Coordinate life skills training, holiday programs, short-term accommodation, and respite services to ensure clients’ needs are met.
- Assist clients through Plan Management and provide guidance on utilising support coordination and psychosocial recovery coaching services.
- Prepare funding proposals and reports for government bodies to secure resources, ensuring continued support for community services and client programs.
- Relevant qualification.
- At least 3 years of experience as a Community Worker or similar role.
- Compassionate and empathetic, with a genuine desire to help others.
- Strong organisational skills, managing caseloads and appointments.
- Effective verbal, written, and interpersonal communication skills.
Salary: This is a full-time position with competitive remuneration between $70,000 - $95,000 p.a. plus superannuation for a 38-hour work week.
