The Opportunity
We are currently seeking experienced Clinical Nurses to join our highly dedicated Referral and Care Support team based at our Keswick head office. In this role you will be at the forefront of the Referral and Care Support team to provide clinical expertise and support to our client experience and ongoing care support services.
The Role
Use your clinical skills, previous referral experience and aged care knowledge to triage referrals and new or ongoing care need requests to appropriate funding streams. Identify, provide support and seamless transitions of clients through services maintaining client centred care. Provide clinical advice to current clients and staff that align with Best Care and Silverchain's mission and values. Provide clinical support and oversight to other members of the Referral and Care Support team.
With the provision of additional leave accruals and penalty payments, the role is part time by negotiation, rotating across 7 days, 365 days - including public holidays, between the hours of 0700 - 2230.
Key Responsibilities
- Utilise your experience to provide clinical support across RDNS Silverchain referral services
- Maintaining contractual compliance and KPIs
- This role supports the provision of quality person centric care according to client needs, contractual obligations and policies and procedures.
- Assist in the resolution of clinical matters within the referral function, as required.
- Actively be involved and contribute to continuous improvement initiatives.
About you
- Current registration with AHPRA and a minimum 4 years post-graduate clinical experience
- Client referral experience in an Aged Care or Community setting is desirable
- Critical decision maker, able to make decision autonomously in complex situations
- Outstanding communication skills (written, verbal and listening) with a friendly and empathetic approach
- Computer literacy, including Comcare (desirable), Microsoft Office suite and referral portals
- Telephone triage and assessment skills
- Demonstrated knowledge of telephone health advice, complex client management, funding programs, community services
