About us Wintringham is a specialised non-religious not-for-profit welfare organisation. We are committed to providing older people who are at risk or have experienced homelessness with a range of high-quality care, housing and services. We employ more than 1,000 staff across Victoria and Tasmania.
Your new role Wintringham is seeking a dynamic and proactive Area Coordinator, to join our multi-disciplinary Hume community team on a Full-Time basis. The Area Coordinator will provide support and supervision for a team of case managers who will work to ensure clients receive the support and care needed based on their funded level of package.
Your duties and responsibilities will include:
- Promote a non-institutional philosophy by recognising and promoting the rights of clients
- Promote consistent growth and viability of the Home Support Program(HCP)
- Develop strong relationships with local services and networks
- Ensure duty of care to both clients and staff is adhered to at all times
- Ensure high quality, comprehensive case management services are provided.
- Provide support and supervision for a team of case managers
- Ensure services being provided are in line with Aged Care Quality Standards
- Maintain a small caseload of Home Care Package clients
We are looking for candidates with the following:
- An approved tertiary qualification in Social Work, Nursing, Welfare, Community Services or Allied Health.
- Demonstrated well developed networking skills
- An understanding of the aged care sector and community services
- Current Driver’s License
- Experience in case management supervision and staff development
- Experience in service delivery in a community setting
- Sound understanding of the aged and community services legislative frameworks
- Experience in the Health or the Aged Care sector and Homelessness
- Commitment to the organisation and its philosophy of social justice.
- Well-developed communication and liaison skills with a variety of people from varying socio-economic backgrounds
