About the Role
We are currently seeking an experienced, energetic Home and Living Coordinator to support independent living for our participants in Yarrawonga. This position reports to the Service Coordinator and is accountable for the oversight, delivery and continuous improvement of high-quality person-centred services to our participants, aligned to ConneXtions Mission, Vision and Values.
The Home and Living Coordinator ensures our participants receive high quality individualised services that enable them to successfully achieve their goals & desires. To provide direct participant support in line with ConneXtions Values and Vision, by walking alongside participants in their individual journeys, empowering and supporting capacity building outcomes to assist participants to reach personal goals and outcomes, whilst ensuring their access to active participation as values members of the communities in which they live.
This position and duties may vary depending on the specific needs of participants and workforce members. Common day to day responsibilities and duties of the position may include (but are not limited to):
- Proactive collaboration with ConneXtions Service Coordinators and the direct support team
- Overseeing meal plans, shopping, medication administration and documentation
- Ensuring that the home is compliant, and delegating tasks to ensure that it is always clean and presentable
- Liaising with families and allied health as required and working co-operatively them to ensure the best interests of the participant are served.
- Ensuring all participant’s appointments are booked and completed
- Advocate for Participant, practise person centred care and always maintain participant privacy and dignity.
- Help to sustain the persons support network including family and friends.
- Ensure that support is linguistically and culturally appropriate to the needs of the people we support.
- Value and respect the diversity of cultures in the process of implementing Support plans.
- To work safely and participate in relevant Work Health and Safety processes, including Hazard identification and Risk Assessment
- To report any incidents or complaints as per NDIS Quality and Safeguards Commission and Connextions policy in the Client Information Management System
- To comply, at all times with all requirements, as outlined within the NDIS Quality and Safeguards Commission’s The NDIS Code of Conduct – Guidance for Workers Scope of Practice.
- To demonstrate a commitment to ongoing learning and participate in professional development training, as provided and negotiated.
- Always ensure that case notes are completed and maintained in an accurate and timely manner and that the participants support needs and plans are read and understood.
About You
A Home and Living Coordinator should be able to demonstrate they possess the following skills:
- Minimum Certificate IV in Disability Support, or a relevant tertiary qualification.
- Solid understanding and working knowledge of issues facing people with disabilities and their families.
- Demonstrated superior understanding of social inclusion, the enabling factors and barriers to social inclusion for people with disability, their families and carers.
- Efficient time management skills and the ability to manage multiple tasks
- As a leader and mentor, you are committed to inclusion, capacity building, community engagement, and the delivery of high-quality services for people with a disability and respect the rights of all individuals.
- Good communication skills (written and Verbal) and a commitment to engaging positively with others
- Demonstrated ability to work and function as an effective team member and work independently.
- Understanding and aligning your work ethic with the direction and values of the organisation and the disability sector
- Computer literacy – Ability to be able to access Microsoft Office, and complete relevant records in the organisation's client management software, with a high level of accuracy & attention to detail
