About the role
The Tierney House Residential Support Worker is responsible for delivering high-quality, compassionate care to residents, ensuring their physical, emotional, and social well-being, and supporting their journey out of homelessness.

Provide 24-hour care and supervision for residents, ensuring a safe and supportive environment for health and well-being.

Collaborate with the Homeless Health Service and other partners to enhance health outcomes and connect residents with vital services and resources.

Conduct comprehensive assessments and care planning for residents, coordinating referrals to other services as needed.

Maintain accurate documentation and electronic records, ensuring clear communication of care plans and progress.

Promote resident integration into the community and support their transition to mainstream healthcare and services.
About you
We are seeking a compassionate and dedicated individual who is committed to providing high-quality support to residents in need.

Experience supporting residents experiencing homelessness, ideally within a residential setting or Specialist Homeless Service.

Strong verbal and written communication skills, with the ability to engage effectively with residents, families, staff, and stakeholders.

Ability to work independently and as part of a collaborative team, demonstrating initiative and responsibility.

Proven experience in developing and implementing care plans to assist residents in their daily living activities.

Cultural competency and experience working with diverse populations, including those with co-occurring mental health, substance use, and physical health issues.
