We are looking for an experienced and dedicated Occupational Therapist to join our multidisciplinary team within the Central Coast Transitional Aged Care Program (TACP). The TACP provides short-term, goal-oriented therapy services for older individuals following a hospital stay, assisting them in transitioning back to their homes and maintaining their independence.
As an Occupational Therapist in the TACP, you will work with clients in both community (Transpac) and residential (Transitional Care Unit) settings to deliver high-quality, client-centered therapy aimed at restoring function, optimising independence, and facilitating longer-term care arrangements as required.
An Eligibility List (E-List) or talent pool may be created from this recruitment episode to assist in filling future permanent, temporary, or casual full-time and part-time positions.
This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks’ notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.
Key Responsibilities
Work collaboratively with clients, families, and other healthcare professionals to develop and implement individualised therapy plans.Provide expert Occupational Therapy services to clients in both community and residential care settings.Focus on restoring clients’ functional capacity, enhancing their independence, and helping them transition back to their homes.Assist clients in completing the restorative process and accessing longer-term care arrangements where necessary.Contribute to the development and implementation of evidence-based practices to achieve optimal health outcomes.Collaborate with other senior therapists and team members to ensure coordinated and effective service delivery across all TACP sitesSupport the delivery of high-quality, time-limited, and goal-oriented therapy as part of the TACP.
