Primary Purpose of the Position
As a Specialist Support Coordinator , you will play a pivotal role in assisting individuals with disabilities to navigate the NDIS system, ensuring they make the most of their funding. There may be a need to support some participants who do not have specialist funding, as directed. You will collaborate with clients, families, and service providers to set goals, connect with appropriate services, and create comprehensive plans that meet the individual’s needs and aspirations. You will be an advocate and guide, helping participants access the services they need for a better quality of life while promoting self-advocacy and independence.
There is the option for work to be full-time or part-time, depending on the successful candidate's preference. We are looking for someone to fill between 1.0 FTE and a minimum of 0.8 FTE.
Key Responsibilities
- Planning & Goal Setting: Collaborate with participants to develop personalized plans that align with their goals and support needs.
- Service Coordination: Connect participants with the appropriate NDIS service providers, ensuring access to necessary supports.
- Monitoring & Review: Conduct regular check-ins to assess the effectiveness and financial sustainability of services and adjust plans as needed.
- Problem Solving: Address and resolve issues with service providers, ensuring that participants receive the supports they need.
- Advocacy: Empower participants to make informed decisions about their care while advocating for their rights and needs.
- Plan Implementation & Documentation: Assist in implementing NDIS plans and maintain accurate records of services, meetings, and progress.
- Stakeholder Liaison: Work with participants, families, and external service providers to ensure coordinated, holistic support.
Key Requirements
- A relevant degree in Human Services, Allied Health, Teaching, or a similar field.
- Lived experience with disability or navigating the NDIS system is highly desirable.
- Ability to work with contemporary information and communications technology in an outreach service environment.
- Flexibility to travel to appointments as required.
- Excellent communication, problem-solving, and advocacy skills.
- Strong understanding of the NDIS system and the ability to navigate its complexities.
- A person-centred approach with a commitment to supporting independence and improving the quality of life for participants.
