The Role
We are looking for an experienced Nurse (Enrolled or Registered Nurse) to join our Royal Park Medical Centre to be the driving force and advocate for the local community in preventative health. This role will see you support GPs and their patients to identify and manage their chronic conditions and ultimately keep them out of hospital. Preferably full time but happy to discuss part time options with the right candidate.
The Tasks
- Prepare Care Plans and Team Care Arrangements to help patients manage their disease/s
- Empower patients to increase ownership over their health
- Coach patients on embedding health behavior change principles
- Enable patients to access ongoing care from a multi-disciplinary team
- Assist in early identification of chronic disease by conducting health assessments
- Work with GPs to promote preventative healthcare throughout the practice
- Increase the number of patients who receive a care plan for the first time
- Ensure patient’s care plans are reviewed regularly
- Manage administrative tasks relating to carrying out CDM appointment

The Successful Candidate
- AHPRA registered Enrolled or Registered Nurse
- Proven prior experience in Chronic Disease Management and health assessments
- Knowledge of Health Assessment is highly regarded
- Strong organisational and time management skills
- Outstanding communication and customer service skills
- A passion for people and a personable approach
