The Role:
The Home Care Service Delivery will actively support Home care Packages Team, by providing to build open and respectful relationships with our consumers and their representatives and to treat every interaction with a consumer as an opportunity to bring respect, life, and joy to the relationship. The position undertakes a leadership and supervising role in the management of Scheduling Officers, Intake Officers, the Personal Support Worker Coordinator and Relationship Officer including management of the Mill Park Offices.
To be successful for this role you must possess:
Demonstrated experience in building positive relationships with a broad range of internal and external stakeholders that contribute to high quality service provision for clients
Demonstrated ability to work autonomously under limited direction
Must have strong understanding of My Aged Care, retrieving codes and support plans for the provision of service under CHSP
Highly motivated with strong organisational, interpersonal and problem-solving skills with a strong attention to detail
Excellent communication skills – both written and verbal (essential)
Strong client relationship focus
Experience, capability and capacity to lead teams
Desirable Qualification
Familiarity with systems such as Carelink, Procura, or similar client management software.
Proficiency in languages other than English can be advantageous.
Understanding of the aged care and disability sectors, including regulations and compliance requirements.
Additional Requirements
A current NDIS Worker screening Check or willing to undertake
Current Drivers license
Working with children’s check or willing to undertake
Proof of Covid vaccinations
CPR and First Certificate
