Position Title : Care Coordinator – Mental Health
Location: Mandurah Peel Health Hub
Contract Type : Part-time, 30 hours per week (Fixed-term until 30.06.2026, with potential for extension based on funding)

We offer Clinical Care Coordination, GP, and nursing services to ensure individuals receive the right care, at the right time . Our qualified professionals serve as a trusted, confidential point of contact for clients, working with them to develop personalised care plans, initiate treatment, and connect them to both internal and external services. Role Overview
We are seeking a compassionate and experienced Care Navigator to join our team in a dynamic, part-time role. This is a unique opportunity for someone who thrives on making a real difference in people’s lives. In this position, you will be responsible for working directly with clients, assessing their needs, creating personalised care plans, and providing ongoing coordination to ensure seamless access to internal and external support services.
Key Responsibilities
- Partner with health service providers to identify and assist clients facing complex mental health and AOD challenges, developing comprehensive and personalised care plans.
- Carry out detailed assessments to understand clients’ health, wellbeing, and social support needs, and take the lead in connecting them with necessary services.
- Coordinate the delivery of therapeutic interventions, liaising with both internal and external providers to ensure a streamlined approach to care.
- Support clinical governance, assisting in clinical reviews and maintaining adherence to established clinical processes and standards.
What We Are Looking For
To succeed in this role, you will need the following:
- A tertiary qualification in mental health, such as:
- Social Worker with mental health experience
- Occupational Therapist with mental health experience
- Mental Health Nurse
- Registered Psychologist (either provisional or fully registered)
- Active registration with your professional body
- A current WA driver’s license
- A clear police check
- A Working with Children Check (WWC)
- Legal right to work in Australia
