Hours: Monday to Friday 8.30 to 5pm Actual days to be negotiated but must include Friday. There may be a requirement to work extended hours on evenings and weekends in the future in line with service demands.

About the opportunity

Mercy Mental Health & Wellbeing Service is a provider of older adult mental health services in the Wyndham and Hobson’s Bay communities. This is an exciting opportunity to be part of a team that provides outreach treatment and support to older adults living in their own homes or within residential aged care who experience moderate to severe mental illness along with complex physical health concerns and/or frailty.

Our team includes Consultant Psychiatrists, a Psychiatry Registrar, Mental Health Nurses, Social Worker and workers who have had their own Lived Experience of mental health issues.

The Mental Health Clinician works collaboratively with the multidisciplinary team to actively engage older adults in assessment, treatment and will assist them, their carers and families to access other appropriate supports and services in the community.

What you’ll bring

  • You will work with consumers, their carers and or families to develop holistic comprehensive treatment and recovery plans

  • You will provide information and advice about living with mental ill health

  • You will advocate for and support social inclusion

  • You will work alongside other specialists involved in the care of consumers, for example residential aged care staff or GPs

  • You will work within a recovery framework providing evidence-based practice interventions as part of a collaborative multi-disciplinary team

To thrive in this role, you will have:

  • 3 years minimum mental health work experience (Ideal)

  • Registration with AHPRA as a Registered Psychiatric Nurse with successful completion of a Post Graduate Psychiatric/Mental Health Nurse qualification Registration with AHPRA as an Occupational Therapist or recognised degree in Social Work and eligibility for membership of the Australian Association of Social Workers.

  • Knowledge and experience working with older adults who experience mental ill health and or a neurodegenerative condition

  • Demonstrated clinical competency in mental state and risk assessment together with the ability to prioritise workload and coordinate clinical care with other service providers

  • Self-motivation, flexibility and the ability to work autonomously

  • A current full unrestricted Victorian Drivers Licence, with the ability and willingness to drive a Fleet vehicle

  • Full Australian working rights

  • The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.

You will also be required to provide evidence of, or in the process of obtaining:

  • A current Police Record Check and Working with Children Check.
