The Support People is a leading registered NDIS provider in regional Victoria, creating an inclusive world where people with disability live the life of their choosing with our support. The Support Coordinator will support NDIS participants, their family and carers in identifying and connecting with appropriate providers of supports with a view to overcoming barriers, developing skills and achieving the objectives of their NDIS Plan.

You will:

  • Provide strong and clear leadership to the Support Coordination practice. Have a sound understanding of Level 3 Support Coordination participant requirements Assist and oversee the day-to-day activities of Support Coordinators and assistants.
  • Build and maintain relationships with key external stakeholders, particularly Local Area Coordinators (LACs). Support NDIS participants to implement their NDIS plan, including assistance to identify and select providers. Work creatively to find positive outcomes
  • Support NDIS participants to implement their NDIS plan, including assistance to identify and select providers.

  • Deal with crises and report to relevant stakeholders.
  • Optimise participant plans to ensure participants get the most out of their funded supports and plan expenditure remains within budget.
  • Strengthen and enhance participant abilities to self-direct their supports.
  • Assist NDIS participants in planning ahead of time to prepare for their NDIS plan review.
  • Ensure clear, accurate, and timely record keeping.
  • Report any areas where Support Coordinators may require additional training.

What you will need:

  • A relevant qualification in community services, counselling, welfare support, disability support or social work.
  • Case management experience or transferable experience in human services industries.
  • Knowledge of community resources and disability rights issues.
  • Knowledge of the NDIS and at least 3 years of demonstrated experience with NDIS Specialist Support Coordination.
  • A professional and ethically minded approach to supporting people with disabilities; and commitment to supporting and promoting the individual needs, skills, abilities, and personal goals of people with disabilities.
  • A drive to contribute to the success of The Support People and go the extra mile for our participants.
  • Excellent verbal and written communication skills.
  • Good knowledge and experience with Microsoft Office and database applications.
  • Excellent active listening skills and ability to communicate with politeness, empathy, and tact.
  • Management experience.

Screening and external authorisation requirements:

  • Employee Working with Children Check
  • NDIS Workers Screening Check
  • Applicants who have lived overseas for 12 months or longer during the past 10 years are required to provide the results of an international police check.
  • Right to Work in Australia
  • Current Car Licence
  • Your own vehicle (registered & insured)
  • A current First Aid Certificate & CPR Certificate
  • Certificates of completion of the following mandatory NDIS training modules
  • New Worker NDIS Induction Module
  • Worker Orientation Module: Quality, Safety and You
  • Supporting Safe and Enjoyable Meals
  • Supporting Effective Communication
