About the Role
We are seeking a dedicated Clinical Nurse (CN) to join the BRIGHT (Bringing Responsive Integrated Health Care to Our Community) team , delivering expert clinical interventions and specialised nursing support within the CALHN Integrated Care Team . This role plays a vital part in facilitating early discharge from acute care settings and supporting hospital avoidance through comprehensive triage, assessment, health case coordination, and direct clinical care. Operating within both clinic and outreach environments, you will provide acute and sub-acute health responses for clients with complex health needs.
BRIGHT operates out of Sefton Park + Woodville, this role may be required to work across both sites
As part of a multidisciplinary team, you will collaborate with Government and Non-Government organisations to ensure integrated, high-quality patient care. Your responsibilities will include supporting worker health and safety, participating in quality improvement initiatives, and meeting accreditation standards. Additionally, you will provide guidance and professional development support to less experienced staff, ensuring excellence in clinical practice. About You
We are looking for a highly skilled and adaptable Clinical Nurse (CN) who is passionate about providing high-quality patient care and supporting hospital avoidance initiatives. To be successful in this role, you will have:

Advanced Clinical & Assessment Skills – Demonstrated competence in comprehensive clinical assessment and intervention, with expertise in acute, sub-acute, and community-based care. Accreditation in peripheral intravascular device insertion and venepuncture is desirable.

Strong Communication & Problem-Solving Abilities – Excellent written and verbal communication skills, including IT literacy. Ability to work collaboratively within a multidisciplinary team, resolving conflicts and negotiating effectively to achieve the best patient outcomes.

Leadership & Time Management – Proven ability to lead and mentor student nurses and less experienced registered nurses. Strong organisational skills, with the ability to manage time effectively, prioritise workloads, and work under minimal supervision while maintaining accuracy and meeting deadlines.

Comprehensive Knowledge & Experience – At least three years of post-registration nursing experience, with a deep understanding of healthcare legislation, standards, and quality improvement systems. Experience in discharge planning for complex patient groups and a strong grasp of contemporary nursing practices.
