About the Role

We are seeking a dedicated and proactive Emergency Department Liaison Nurse (EDL) to join our dynamic team. In this role, you will provide high-quality nursing services across diverse healthcare settings, continuously building on your expertise through experience and further study. You will play a crucial role in delivering patient-centred care, working towards proficiency in your practice while supporting and mentoring less experienced staff. As part of your responsibilities, you will contribute to the leadership and operational support of the ward/unit/service, working closely with the Advanced Nurse Unit Manager or equivalent to uphold best practice standards.

As an EDL Nurse, you will be responsible for identifying patients, particularly older adults, who do not require hospital admission and facilitating their safe and supported discharge. This includes collaborating with ED medical and nursing teams to develop comprehensive patient care plans and ensuring appropriate community and residential support services are in place. Your role will involve conducting thorough assessments of patients' physiological, psychological, and social needs while liaising with general practitioners, allied health professionals, and community service providers. This is a unique opportunity to make a meaningful impact by enhancing patient flow, reducing unnecessary hospital admissions, and improving continuity of care for vulnerable populations.

About You

We are looking for a dedicated and skilled nurse who is passionate about delivering high-quality, patient-centred care. You will have strong clinical expertise, leadership skills, and the ability to collaborate effectively within a multidisciplinary team.

  • Qualifications & Experience – Registered or eligible for registration as a Nurse with the Nursing Board of Australia, with a current practising certificate. Minimum of 3 years post-registration experience or current classification as a Clinical Nurse.

  • Skills & Abilities – Strong communication, problem-solving, and conflict-resolution skills. Ability to prioritise workload, adapt to challenges with creativity and flexibility, and provide leadership and mentorship to less experienced staff.

  • Knowledge – Understanding of hospital avoidance and early discharge programs, relevant legislation, healthcare standards, and quality improvement processes. Familiarity with the role of an Associate Advanced Nurse Unit Manager.

  • Commitment to Care – Dedicated to providing consumer, client, and family-centred care with a focus on improving patient outcomes through collaboration and innovation.
