The Department
The Caulfield Community Health Service (CCHS) provides a range of clinical and health promotion services for people of all ages who live, work or study within the local community. These multi-disciplinary services are provided in the centre, community, home or via Telehealth. CCHS operates from the Caulfield Hospital site.
The Role
We have an exciting opportunity for a Health Promotion Coordinator (HPC) to join our Community Health Service responsible for leading and coordinating health promotion services accross CCHS.
The position will work as a member of the Population Health Team and will collaborate strongly with members from other teams within the organisation to undertake high quality priority setting, strategy planning, implementation and evaluation. The HPC will receive support, guidance and direction from the Population Health Team Leader across all areas of responsibility. The HPC will provide professional supervision and support to CCHS Health Promotion Officers.
Experience and Qualifications Required
- Tertiary qualification in Health Promotion
- (Bachelor of Science (Health Promotion), Bachelor of Public Health Promotion, Bachelor of Public Health and Health Promotion, Bachelor of Health Science (Health Promotion), any other undergraduate or postgraduate qualification in health promotion, or any other undergraduate or postgraduate qualification relevant to the field of Health Promotion)
- Demonstrated experience in health promotion priority setting, planning, implementation and evaluation
- Demonstrated experience with contemporary health promotion theory and practice, including complex systems and settings-based and place-based approaches
- Ability to work directly with community members and ensure effective community participation
- Demonstrated experience in providing workforce support, training and community education
- Demonstrated commitment to continuing professional development
- Capacity to understand and adhere to organisational policies and guidelines
- Excellent communication, presentation, collaboration, problem solving and information technology skills
- Current Victorian driver’s licence
