We have partnered with one of Australia’s leading Aged Care organisations, and this is an exciting opportunity to join a team that demonstrates professionalism and passion for Home Care. The organisation is expanding its reach and positively impacting the local communities it serves.
In this vital role, you will utilise your experience in Case management to lead a team of Case Managers across Tasmania and Sydney. You will be a crucial member of the leadership team and will benefit from a supportive and engaging workplace culture.
The Role:
  • Lead a team of Case Managers based in Tasmania and Sydney, providing mentorship, guidance, and professional development to cultivate a positive workplace environment.
  • Assist the General Manager in implementing and enhancing guidelines and policies that align with Aged Care Quality Standards.
  • Develop and implement staff training programs and continuous learning initiatives to improve service delivery.
  • Promote a client-centered approach and ensure the team adheres to best practices in aged care.
  • Manage a small portfolio of Home Care Package clients.
  • Support the growth of services by identifying and implementing new opportunities for client engagement and service improvement.
  • Collaborate with internal and external stakeholders to create innovative and effective service delivery models.
About you:
  • Proven leadership skills to inspire and develop high-performing teams
  • Strong communication, negotiation, and problem-solving abilities
  • Background in aged care, especially in Case Management within Home Care services
  • Ability to analyse data, identify risks, and drive improvement
  • Commitment to quality service and passion for aged care
  • Valid working rights in Australia (Passport, Birth Certificate, etc.)
  • Police Check (within the last 3 years)
  • Evidence of COVID-19 vaccinations (3 doses)
