Remote Area Nurse - Chronic DiseaseWe are seeking an experienced and passionate Remote Area Nurse to lead our Chronic Disease Health Program for a large Aboriginal community in the Northern Territory. This is a unique and rewarding opportunity for a skilled nurse to make a real difference in the health and wellbeing of a remote community.
Role Overview: As the Chronic Disease Nurse Program Lead, you will coordinate and work in partnership with a multi-disciplinary team to provide high-quality prevention and treatment services. You will lead the chronic disease team, manage clients, and provide adult health checks for individuals at risk of developing chronic diseases. You will also refer patients to the appropriate allied health professionals to ensure comprehensive care.
Essential Criteria:Minimum of 5 years of broad nursing experience within an acute health setting2+ years of experience working in a primary health-related field in a remote contextHigh-level clinical assessment and problem-solving skillsStrong interpersonal communication and conflict resolution abilitiesUnderstanding of the principles of primary health care, with a focus on the social determinants affecting Aboriginal and Torres Strait Islander populationsUnderstanding of Aboriginal culture or a willingness to learn and live in a remote community
Desirable Qualifications:Postgraduate qualificationsCurrent Advanced Life Support certificationCurrent Remote Emergency Care (or equivalent) certificationKnowledge of Vaccination protocols (or equivalent)
