Whether you're an enrolled nurse ready for a change or someone with an Aged Care background craving a more balanced and supportive work life, this is your chance to use your clinical skills in a rewarding, full-time role. You’ll assess Home Care Package clients, develop individualised care plans, and help older Australians live well at home, without the hustle of hospital life.
What You’ll Do
• Conduct assessments for new and existing Home Care Package (HCP) clients
• Create personalised care plans that reflect client goals, preferences, and risk factors
• Use a wellness and enablement lens to recommend supports and services
• Identify high-impact and high-prevalence risks using structured clinical questioning
• Maintain accurate documentation in our digital care management systems
• Provide support and education to clients and their families
• Collaborate with internal teams to ensure high-quality, person-centred care

About You
You're a compassionate professional looking to transition from hands-on care into a more strategic, structured environment.
We’re looking for:
• Current and previous enrolled nurses, allied health professionals, or individuals with strong aged care knowledge
• Experience with HCP, aged care services, or NDIS (highly desirable)
• Confidence using CRM systems and Microsoft 365 (Excel, Teams, SharePoint)
• Great communication, decision-making, and time management skills
• A calm, thoughtful approach to working with older people and their families
