The Department for Child Protection is seeking multiple Stability Post Care Practitioners to join the Out of Home Care team on a full-time term basis.
In this role, you will be responsible for:
- Providing professional support and assistance of relevant services available for young people transitioning from care.
- Enhancing family relationships in a culturally appropriate manner, including providing information and support to carers.
- Delivering professional guidance and support to DCP staff and clients regarding the SFBC program.
- Developing and maintaining strong working relationships with a broad range of stakeholders.
To be successful in this role, you will have:
- Knowledge of the child protection out-of-home care sector and the needs of young people transitioning from care.
- Exceptional problem-solving skills and the ability to resolve issues during interaction with young people.
- Knowledge and understanding of the issues impacting on young Aboriginal people and families.
- Excellent written assessment and communication skills.

Special Conditions
Essential:
AHP: A degree level qualification in Social Work which gives eligibility for full membership of the Australian Association of Social Workers.
PO: Appropriate degree qualification in Community Services, Social Sciences, Human Services, Health or related field.
Persons of Australian Aboriginal or Torres Strait Islander descent, who have the appropriate background and skills but do not have the essential qualification, may apply for and be engaged/assigned to the role of Social Worker and will be entitled to apply for any Allied Health Professional roles requiring a qualification in Social Work within the Department for Child Protection (DCP).
