This role requires a unique individual who wakes every day to put passion to purpose! Partnering with people in their homes, this role provides clinical assessment to identify meaningful goals that are person centered and develops home based therapy programs to build and maintain independence. With a diverse and varied caseload including geriatric, neurological and cardio-respiratory fields, the role includes continued research, learning and applying best practice to achieve quality care and outcomes including goal-oriented short term restorative care. The role includes providing coaching, training and guidance to our support care team and has plenty of autonomy! You will be based at Rockingham covering the Rockingham and Mandurah Region with company vehicle available for your use.

Along with wanting a great team of peers that you can draw on for support in those challenging moments, collaborative with and who share your pride in your work, your key experience will include:
- Relevant qualifications and registration (AHPRA)
- Prior experience applying your skills as a physiotherapist, ideally in a community-based setting
- Experience in assessment and developing home-based program with the ability to make sound clinical decisions and operate independently
- Fantastic communication skills with a wide range of people, with a love of coaching others
- Curiosity, initiative, problem solving and sharing knowledge with your team
- Knowledge of the Aged Care Act (1997) is highly valued
- Current Driver’s Licence
- National Police Clearance (obtained within the last six months)
