About the role
This is a part-time (0.8) fixed term opportunity for 6 months, based in Battery Point, TAS. Reporting directly to the Team Leader Client Services, you will be responsible for:
- Facilitating individual and group education, information, and support to people living with dementia, their families, and carers.
- Supporting clients in navigating aged care, disability, and health services by providing referrals and guidance.
- Facilitating connections between people living with dementia and their carers through support groups and other engagement opportunities.
- Ensuring service delivery is person-centered, empowering clients to take an active role in their care.
- Contributing to continuous improvement by identifying gaps in service delivery and sharing insights with leadership.
- Building and maintaining partnerships with community organisations and service providers to enhance support options.
- Maintaining accurate client records and data in compliance with professional and legal standards.

About you
To be successful in this role, we are looking for someone with the following:
- Formal qualification in a related field of either Social Work, Allied Health, Nursing and/or significant experience working with clients in the delivery of a community-based service or within a health-related field.
- Demonstrated experience and understanding of the impacts of dementia for individuals, carers, and their support networks.
- Demonstrated experience and knowledge of group facilitation and group dynamics. Experience in facilitating individual and group work with clients in a community setting is desirable.
- Embraces new ideas and change initiatives.
- Achieves results through a commitment to quality, planning and efficient resourcing.
- Communicates clearly and openly with different audiences.
