Community Support Worker/Care Manager

About the role
ATN is seeking an experienced and compassionate Community Support Worker/Care Manager to join our team in Perth, Western Australian. As a full-time Community Support Worker/Care Manager, you will play a crucial role in ensuring that our disability and aged care clients receive the highest quality of care and support to help them live independently in their own homes.
What you'll be doing
• Conduct comprehensive assessments to identify the unique needs and goals of each client
• Develop and implement individualised care plans in collaboration with clients, their families, and other healthcare professionals
• Coordinate and monitor the delivery of a range of in-home support services, including personal care, domestic assistance, and social activities
• Advocate for clients' rights and ensure they receive the appropriate level of care and support
• Regularly review and adjust care plans to adapt to clients' changing needs
• Maintain accurate and up-to-date client records and documentation
• Collaborate with other members of the multidisciplinary team to ensure seamless service delivery
What we're looking for
• BILINGUAL – MUST HAVE STRONG COMPREHENSION WITH VIETNAMESE AND ENGLISH
• Tertiary qualification in social work, nursing, or a related field
• Minimum 2 years' experience in case management or a similar role in the aged care or community services sector
• Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, their families, and other stakeholders
• Strong problem-solving and decision-making abilities, with a focus on client-centred care
• Thorough understanding of aged care legislation, policies, and best practices
• Proficient in using client management systems and Microsoft Office applications
• Compassionate, empathetic, and committed to promoting the independence and wellbeing of older adults
• Access to a reliable, registered motor vehicle with full comprehensive insurance and a valid driver's license
• Strong computer skills
• National police check and working with children check upon role offer
